what about a update model of

encounter (only change time, location here)

      0_* health_issue ( or new health_issue)
      1_* clin_root_item ( view changes on type of clin root item. 
root_items can be
in unordered list regardless of SOAP category . If P views, have Pmed, 
Pvacc, Prequest, Preferral ).

Have another panel for viewing received items ( documents, images, 
results ) also arranged in
a selection tree, with another panel next to it for viewing the detail 
(with an expand button
that can size it to the whole screen, and then back down again).
