do you put "Undisclosed" just on suspicion, or do
you at least get affirmation from the patient that there is at least

I would try, yes.

Once you share your view of the above, could you then share how you
would handle the situation where the patient does recall pretty
confidently that they did indeed, in the past, have a problem with
one of their previous medications, but simply cannot remember the
medication, but they do or don't clearly recall the adverse reaction?
I would enter an allergy - after all very likely there IS one - with an unspecific name like
trigger = "some previous medication".

So it seems we go from

        Unasked

to either

        None (meaning as far as could be determined with patient co-operation)

or any combination among

+-      Specific allergies

+-      Something the patient cannot clearly-enough recall
                (but we should put in what we can)

+-      Something the patient prefers to withhold *
                (but if patient acknowledges withholding
                        e.g. for personal reasons
                        we should put in a comment to this effect)

                * we should consider that the patient *could* be
                        willing to share the information at a future
                        visit, especially depending on the nature
                        of future complaints or of Rx we would offer
