Is the schema and middleware such that an inbox message like

        personal        FYI     Mr Kirk is off to Galactica until 2009

despite that it happens to not be linked to Kirk, *could* be linked to Kirk, in 
other words support for even non-clinical messages to still be attached to a 
patient?
