When entering a vaccination you either know the vaccine or you don't.

In case you know the vaccine it either exists in GNUmed or not.

If not you create it by means of the vaccine management UI.

Then you select the vaccine and enter the remaining items. You
certainly don't worry about indications because they are defined
  by the vaccine you selected.

In case you do not know the vaccine (name) you only know the indications
that were vaccinated agains.

Now there is either a generic vaccine already in GNUmed for that
indication in which case you can select it just like any other
  vaccine.

  Or there is not.

This is particularly obvious when there was a combi-indication
vaccination for which you don't know the vaccine name. So you
tick off the indication (INSTEAD of selecting a vaccine) and
GNUmed will factor out one vaccination record for each indication
    under that indications generic vaccine.

The Indications field will only be active when there is no vaccine
in the Vaccine field. If there is it will be deactivated and show
the indications the vaccine is configured for.
