Hello:

I have updated the below report template fodt files:

   1. health
   2. health_dentistry
   3. health_imaging
   4. health_lab
   5. health_nursing
   6. health_reporting

The changes are main:

1. Make templates simple and easier to read.
2. Clean all direct format in fodt files.
3. Clean string split abnormal.

I would like others test them and feed back, so I can know this change
will be accepted or not, and find new bugs when I changes.
