Well, but they are in a hidden directory ".gnumed" which was the
intention.

Are the files *supposed* to be hidden?

No - just living in a hidden directory.

Are users not intended to be modifying their config file ~/.gnumed/gnumed.conf or are they intended to be able to know or figure out that these are hidden, but that it is OK to modify them anyway?

Putting the files in a hidden directory has only one reason: Keep
people away who are not knowing what they are doing.  You can not
hide any information from other people.  It's more like: Do not
make Joe Normaluser curious about these strange logfiles / config
files if he is browsing the file in his home directory.  Not more
not less.
